Online Reporting Form

This form is intended to address concerns in four primary areas:

This form is NOT a police report. If you are in immediate danger, please call 9-1-1.

The primary goal of this reporting form is to provide support and resources to individuals or communities impacted. This reporting form will also be used to evaluate whether further investigation is required for potential violations of University policies.

The submitted form will be distributed to the Office of Compliance; the OC will work with the Office of Student Assistance and Support and/or Human Resources to appropriately address each concern. The university highly values privacy, and only “need to know” information is shared with appropriate contacts in order to implement necessary university response.

Reports may be anonymous; however, anonymous reports may limit the University’s ability to respond to the incident.

Please be aware that our reports may be subject to disclosure under Wisconsin’s Public Records Law. Names of students are confidential, but employee names and other information may be made public if requested. Discrimination, Harassment & Bias

Conduct that adversely affects any aspect of an individual’s employment, education, or participation in an institution’s activities or programs, or has the effect of denying equal privileges or treatment to an individual on the basis of one more characteristics of that individual’s protected status or category

Hostile and Intimidating Behavior

Unwelcome conduct, while engaged in or relating to employment or appointment activities, that does not further the legitimate academic or operational interests of the institution and is conduct that a reasonable person would find is so severe or pervasive that it makes the complainant’s conditions for work inhospitable or intolerable and/or; unreasonably and negatively affects the complainant’s well-being or safety in the workplace and/or; unreasonably interferes with the complainant’s ability to carry out their work responsibilities to the University.
